A copy of an Investigative, Toxicology, and Autopsy report can be purchased from our office.  The cost is $4.75 for each report with an additional charge of $.10 per page. There is an additional $2.00 fee for mailing out a report. The report can be ordered in person or by mail. The next of kin may receive one copy of each report at no charge. Contact the Medical Examiner-Coroner's Office during regular business hours to confirm the availability of the report and cost.  When ordering by mail please include the name of the decedent, date of death, Medical Examiner-Coroner's case number and a check made payable to Santa Clara County Medical Examiner-Coroner's Office.  Mail it to:

Certified Copy of Death Certificates 
Certified copies of Death Certificates should be initially ordered for the family by the funeral director handling the family arrangements.  The Medical Examiner-Coroner's Office does not have copies of death certificates.  Additional copies can be purchased from the Public Health/Vital Statistics department.  Contact them at:
