Opening A Case

You can open a case at the Department of Child Support Services in several ways.

  • You can visit our office at 2851 Junction Ave., San Jose between 8:00 A.M. and 5:00 P.M.
  • You can call us at 408.503.5200, and we will send an application to you in the mail.
  • You can fax a request to 408.503.5252.
  • You can mail a request to our office at 2851 Junction Ave., San Jose, CA 95134
  • You can download the attached application below.

Photographs of children and parents

We will mail an application within five days to anyone requesting one by phone or from the date we receive the request.

It is important to include in your application as much information as you can about both parents and the child. The more information you provide, the faster DCSS can obtain your child support order. Information that identifies the noncustodial parent (NP) is crucial. It’s a good idea to take the time to find the NP’s social security number, driver’s license number, address, and place of employment.
