Local post offices, libraries, city and county government offices.
The California Secretary of State provides an on-line form. After completing the form, the Secretary of State will print and mail the form to you.You must sign the form and mail to the Santa Clara County Registrar of Voters.

Complete the Vote by Mail Ballot Request Form available below or on the back of your Sample Ballot Pamphlet.

Write a letter requesting a Vote by Mail Ballot. Include your name, residence address, mailing address (if different) and your signature. Mail or fax to: Vote by Mail, PO Box 1740, San Jose, CA 95109. Fax: (408) 293-6002.
