| Division Director: (Acting) Teea Gilbert, MFCC Adult and Older Adult System of Care provides mental health services to adults ages 18-59, and older adults who have a serious mental illness. Outpatient services are provided through community based Service Teams that are geographically spread throughout the county and are available24 hours a day, 365 days a year. Additional programs offered to this population include: Crisis residential and transitional residential programs, day rehabilitation and day socialization services, a jail diversion program, client advocacy services, supported housing, shelter programs for the homeless mentally ill, vocational and educational supports and a representative payee program. There is a limited number of Service Team openings for the dual diagnosed mentally ill (mental illness and developmentally disabled) and for deaf and hearing impaired individuals with a mental illness. Acute inpatient hospitalization, and long term care services are available.

| Health Program Specialist: Richard Rodriquez Similarly to the other county mental health facilities, the Service Team at the West Valley site serves the seriously mentally ill of our community, providing Case Management, Psychotropic Medication and the Rehabilitative services that are designed to help our clients improve their quality of life. In addition, our team provides specialized services to the Dually Diagnosed population (individuals with concurrent mental illness and substance abuse disorders), utilizing a clinical model that integrates both mental health and substance abuse treatment for a more comprehensive approach to this high risk group of people. | Alison Brunner Santa Clara County Mental Health Department has a strong patients rights and advocacy program. This is to assure that mentally ill persons who have been certified for 14 days of intensive treatment are provided information about the commitment process, about their rights to an administrative hearing or a judicial review by writ of habeas corpus and their right to counsel in such judicial review, including the right to court-appointed counsel. The Department contracts with the Mental Health Advocacy Project to perform these functions. The Advocates also monitor Santa Clara County inpatient facilities for compliance with LPS commitment process.

The Mental Health Department provides a supplemental payment support group to contract Board and Care Homes to provide specialized mental health services to clients requiring a higher level of supervision and care in order to live successfully in the community. Clients are seriously mentally ill and experience difficult behavioral or Medi-Cal problems and may concurrently have language and cultural issues. Access to this program is through approval by the Mental health Department 24 Hour Care Unit.
