The Diabetes Center at Santa Clara Valley Medical Center is dedicated to the treatment and care of individuals with diabetes. The goal is for every client to achieve optimal health. Realizing good health and well being for each client is accomplished through understanding the disease, evaluating individual patient needs, and successfully managing the disease. The Diabetes Center staff promotes self-management skills and educates individuals and families about diabetes.
SCVMC considers diabetes education the most important aspect of successful self-management of diabetes. Our group classes cover a variety of topics in diabetes care and are taught by an expert staff. The Diabetes Center staff includes dietitians (RDs), nurse educators (RNs), pharmacists (PharmDs), physician assistants (PAs) and physicians (MDs). Our nursing and dietitian staff includes certified diabetes educators (CDEs) who are specially trained and nationally certified. Our CDEs offer one-on-one sessions, as well as group classes, to teach patients and family members how to take care of diabetes and how to stay healthy. Utilizing hospital-approved protocols and under the guidance of physicians, our educators are trained to initiate and adjust medications to improve diabetes control for our patients.
