Our Mission

The mission of the Palliative Care Program is to improve the quality of life of patients with advanced illnesses. We do this by supporting patient-centered care through a multidisciplinary team who is committed to enhancing the physical, social, psychological and spiritual well-being of the patient and family. All the while, the team remains sensitive to the patient’s cultural traditions, personal goals, beliefs, and values.

The VMC Foundation raises funds to enhance programs throughout the Santa Clara Valley Health & Hospital System, including Santa Clara Valley Medical Center. The VMC Foundation has established a Palliative Care Fund to support this program. Every gift makes a difference and is greatly appreciated. Please call 408.885.5201 for more information.
