This service is for patients of Santa Clara Valley Medical Center and only a patient’s physician can order a formal consultation. Patients and family members may request assistance by asking any member of their healthcare team to contact us.

For formal SCVMC inpatient Palliative Care consultations:

  • Services are currently available Monday through Friday.
  • Physicians should check “Amion” to get the name and pager number of the Palliative Care consultant on call.
