VHP Primary Care Physician & Self-Refer Specialists List  

Self-Refer Clinics 
VHP members may self-refer to the following clinic without obtaining a referral or prior authorization:

Eye Services 
For VHP members with Vision benefits offered by their employer are encouraged to seek services for routine refraction examinations (for eyeglass prescription only) with those vision providers, i.e. Vision Service Plan (VSP).  These vision providers offer many provider locations conveniently located near where you live or work, some with weekend and evening hours and some have eyeglasses/lenses to purchase onsite. 

For VHP members without Vision benefits, please call 408.793.2525 for routine refraction examinations (for eyeglass prescription only). VHP does not cover the cost of eyeglasses/lenses or any copayments for costs incurred through your employer's vision plan.You must have a referral from a VHP provider for all other covered eye services.

Dermatology Services
VHP members may self-refer to a Plan Dermatologist for dermatology covered benefits. For Adults and Pediatrics, call 408.793.2555.

Obstetrics/Gynecology (OB/GYN)
Female members age 18 and over may self-refer to a VHP Network Obstetrician/Gynecologist.
