A Primary Care Physician (PCP) is medically trained to take care of your routine healthcare needs and is the primary person responsible for coordinating your care, including referrals to specialists. PCPs practice in a wide range of medical disciplines and can be Family or General Practitioners, Pediatricians, Geriatricians or Internists (Adult Medicine). Obstetricians /Gynecologists (Ob/Gyns) also serve as PCPs.

VHP encourages you and your family to choose your own personal physician. It is important to establish a relationship with your doctor so they can help you get the medical services you need and play a key role in coordinating the healthcare services you receive.
Included in your New Member Packet is a Primary Care Physician list Simply call Member Services at 408.885.4760 or Toll Free at 888.421.8444 with your choice or indicate your choice on your enrollment application.
Scheduling an appointment with your PCP is simple. For Primary Care Physicians who practice at a Valley Health Center location, call “Valley Connection” at 888.334.1000 and a representative will assist you. You can make appointments through “Valley Connection” Monday to Friday, 8am-9:30pm or 8:30am-5pm on weekends and holidays. If the PCP you choose does not practice at a Valley Health Center location, the PCP list includes an appointment telephone number to schedule all appointments with those PCPs. |
