Sec. A6-152. Elements of periodic report.

Each of the boards and commissions named herein shall provide to the Board of Supervisors every third year, on or before June 30, a report which will contain the following elements:

(1) Statement of original purpose and goals, and any official changes to them which have occurred during the preceding three-year period;

(2) Statement of accomplishments during the preceding three-year period;

(3) List of factors which have aided or inhibited the achievements of these accomplishments, including, but not limited to, the composition and purpose of the board or commission, staff support, and external factors;

(4) Estimated annual costs of operation for each year of the triennial, including staffing costs, source of staffing, services and supplies and other elements of operating costs. The statement of costs shall also include revenues to the board or commission and identify the source of such revenues;

(5) Analysis of the board or commission membership during the triennial as compared to County population characteristics, and an assessment as to whether the required composition of the board or commission is appropriate to its functions;

(6) Statement of objectives for the next triennial; and

(7) Suggested modifications and revisions necessary in membership, organization, purpose and scope of activity, with justification and suggested language to accomplish these modifications.
