Public health nurses are part of the collaborative multidisciplinary team serving In-Home Supportive Services clients. Public Health Nursing Services include the following:
- Long-term care information and assistance in voluntary placement.
- Health consultation and joint home visits with social workers, including collaboration in the assessment of functional level and mental status.
- Personalized information regarding chronic conditions â?? e.g., diabetes, arthritis, multiple sclerosis, Parkinsonâ??s, Alzheimerâ??s, end-stage renal disease, cardiovascular disease, chronic obstructive pulmonary disease, cancer, seizure disorder, etc.
- Teaching/referral related to paramedical services â?? e.g., insulin injection, use of glucometer, skin and ostomy care, feeding tubes, home dialysis, bowel program, bladder care, etc.
- Other teaching related to nutrition (e.g., diabetic, low cholesterol, and low fat diets), range-of-motion exercises/post stroke rehabilitation, incontinence, use of assistive devices, client transfers, medication management, oxygen for home use, communicable diseases (e.g., AIDS, TB), traumatic brain injury, developmental disability, mental illness, radiation and chemotherapy, post surgical conditions (e.g., coronary bypass hip/knee surgery, etc.
- Assistance in navigating the medical care system (e.g., getting a medical appointment, referral to and close collaboration with physicians and other health care providers, such as physical therapists, occupational therapists, podiatrists, etc.).
- Blood pressure checks in conjunction with other services.
- Teaching providers regarding client care, prevention and coping with burnout.
- Referral to other health related community based services and agencies.
