||UPCF Hazardous Waste - Hazardous Waste Tank Closure Certification Form
This form must be submitted and uploaded as a PDF file attachment to the local Unified Program Agency (UPA) electronic reporting portal or the California Electronic Reporting System (CERS) to certify that aboveground and underground storage tanks which previously contained hazardous materials, and which are cut and/or cleaned on-site, have been properly cleaned and rendered non-hazardous. [Note: Submittal of this form is only required to certify cleaned tanks as non-hazardous. It is not required if the tank is is merely rinsed to lessen the risk of explosion during removal and/or transportation off-site.]
