The following are the primary forms used to evaluate a project's environmental impacts, pursuant to the California Environmental Quality Act (CEQA). The Environmental Information Form must be completed for all projects where CEQA review is required (see Permits). More details and instructions can be found by clicking the links below.
Environmental Information Form
To be completed by all applicants for all the land use approvals listed above. The Environmental Information Form contains a list of questions relating to existing site conditions and the proposed improvements, and relation to environmental factors (e.g. riparian, geology etc.) that may impact the proposed project.
Environmental Review Checklist
This document contains a checklist of items required to be submitted with applications that require environmental review.
Petition for Exemption from an Environmental Assessment
To be completed only if you believe that your project falls within a category determined to be exempt from environmental review per State law (California Environmental Quality Act).
Petition for Use of Prior CEQA Document
To be completed only if you believe that a previous Negative Declaration or Environmental Impact Report [EIR] adequately addresses the impacts of the proposed project.
Use of a prior Negative Declaration or Environmental Impact Report [EIR] may be appropriate where it can be shown that a previous Negative Declaration or EIR has been prepared that adequately addresses the impacts of the proposed project and (a) no substantial changes have occurred or (b) no new information of substantial importance has become available since the preparation of the previous document.
Notice of Determination Fee Form
This form can be used to determine the fees associated with required CEQA filing.
