- Bring the completed form (if available), valid identification, and cash or check for $23.00 (or $14.00 government rate) for each certificate to:

Orders will be processed while you wait in most cases.
IMPORTANT NOTE: Another person may bring your request to our office. Note that if that person is not an authorized party, a notarized Certificate of Acknowledgement from you is required as detailed in Obtaining a Birth Certificate by Mail.
