Obtaining a Death Certificate in Person

1. Review the requirements for authorized parties.

2. To save time and ensure you have the required information, you may download and complete the order form prior to your visit.  (See Attachments below).

3. Bring the completed form (if available), valid identification, and cash or check for $21.00 for each certificate to:

Clerk-Recorder’s Office
East Wing, First floor
70 West Hedding Street (at First Street)
San Jose CA 95110

Hours: Monday through Friday (excluding holidays), 8:00 AM - 4:30 PM

4. Orders will be processed while you wait in most cases.

IMPORTANT NOTE:  Another person may bring your request to our office.  Note that if that person is not an authorized party, a notarized Certificate of Acknowledgement from you is required as detailed in Obtaining a Death Certificate by Mail.
