Passport Requirements/Checklist

You now have the option to schedule your appointment online or by contacting our office. Passport appointments are available from 8:30 AM to 2:30 PM Monday through Friday. 

 

Please read through the information below then check the "I have read the Requirements / Checklist" box for the option to schedule appointment online.

Note that the "hand carry" processing option is no longer available.

 

1​ Children under 16 years old

Both parents or legal guardian need to appear in person. Please see U.S. Department web site for allowable exceptions.

Passport photos of children - the child must be able to stand without assistance in order to take picture. If assistance is required, the clerk will not take the picture.​

Passport Application​

Form DS-11 (available at the Recorder's Office, at any post office or downloadable from the US State Department web site.​
3​

Passport photos​

Two identical 2" x 2" photos that must be taken within the last 6 months. Front view with a white or off-white background. (Available for purchase at the Clerk-Recorder's Office.)

Evidence of citizenship

Documents will be submitted with application and returned with passport:

  • A certified birth certificate copy (California and Texas birth abstracts not acceptable)
  • A Certificate of Naturalization or of Citizenship
  • An expired U.S. passport
  • A Consular Report of Birth​
5​

Identification ​

Must have your signature and photograph or physical description:

  • A valid driver's license
  • A U.S. Passport
  • An official military identification card
  • A Certificate of Naturalization or of Citizenship​
6​

Acceptable forms of payment​

Acceptable forms of payment for fees.

  •  Check (one check per applicant)
  • Money Order​
 
 


Processing Options:

​Routine Service 4-6 weeks processing time​
Expedite Service​ 2-3 weeks processing time. Expedite fee required
In-Person Service ​ < 2-3 weeks lead time. You must apply in-person at the San Francisco Passport Agency. Expedite fee required
