How to Report

​Diseases and outbreaks may be reported by phone, fax, or mail, as outlined in the Mandatory Disease Reporting Guidelines.  The links below will take you to the Confidential Morbidity Report (CMR) forms that you can print out, complete, and send by mail or fax.  The last link will take you to a page with instructions to assist you in completing the CMR.

• Confidential Morbidity Report (CMR) form - 110A: for reporting all conditions except Tuberculosis and conditions reportable to DMV.


• Confidential Morbidity Report (CMR) form - 110B: for reporting Tuberculosis.


• Confidential Morbidity Report (CMR) form - 110C: for reporting lapses of consciousness or control, Alzheimer's disease or other conditions which may impair the ability to operate a motor vehicle safely (pursuant to H&S 103900).
