Our mission: The Santa Clara County Child Health and Disability Prevention (CHDP) program is dedicated to the health and well being of children and youth. CHDP provides for the early detection and prevention of health problems among children and youth from low-to-moderate income families. Program staff works collaboratively with health care providers and child advocate agencies to ensure that eligible children receive quality health care and to empower families to be knowledgeable, responsible health care consumers.
Children and youth are eligible if they are:
- On Medi-Cal and 0-21 years old
- Low to moderate income and 0-19 years old (Children and youth may be able to receive temporary Medi-Cal for up to 60 days through CHDP gateway)
