Child Health and Disability Prevention (CHDP)

Our mission:
The Santa Clara County Child Health and Disability Prevention (CHDP) program is dedicated to the health and well being of children and youth. CHDP provides for the early detection and prevention of health problems among children and youth from low-to-moderate income families. Program staff works collaboratively with health care providers and child advocate agencies to ensure that eligible children receive quality health care and to empower families to be knowledgeable, responsible health care consumers.
 
Health Exams for ELIGIBLE Children and Youth
 
 
 
Children and youth are eligible if they are:
  • On Medi-Cal and 0-21 years old
  • Low to moderate income and 0-19 years old (Children and youth may be able to receive temporary  Medi-Cal for up to 60 days through CHDP gateway)  
 
Regular health exams can:
 
  • Help children and youth stay healthy
  • Identify health problems early and refer for treatment as needed. A health problem found and treated at an early age is easier to correct and can reduce or prevent serious problems for the child or youth later in life.
 Types of CHDP Exams:
  • Well- baby and well- child exams
  • Preschool/ Head Start exams
  • 1st grade exams
  • School exams
  • Sport or camp physicals
  • Teen physicals
 
All CHDP exams include:
 
  • A developmental and heal
  • th history
  • Head-to-toe physical inspection
  • Height & weight check, growth assessment
  • Nutritional assessment
  • Hearing and vision screening
  • Oral health screening (does not replace dental exam)
  • Immunization as needed
  • Blood and urine tests
  • Tuberculosis screening
  • Answers to your questions and an explanation of the results of the health exam
If the tests indicate a need for further diagnosis and treatment it is important to follow the health provider’s recommendations.
