| Purpose of the Placement Tool |
|
The purpose of the tool is to determine:
- The child’s immediate and ongoing permanency needs
- The type of placement that will provide the child with the least restrictive setting appropriate to his or her needs.

| Who Completes the Placement Tool |
|
The social worker assigned responsibility for ongoing case management completes the Placement Tool. This includes:
- DI social workers
- VFM social workers (for VFR cases)
- Continuing social workers

Also refer to page 79 of the CAT User Guide.
- Names and birthdates auto populate.
- One tool is completed for each child in the case.
- The Placement Assessment (PA) Tool is completed:
- When a child must be placed outside the family home to ensure his or her safety.
- The tool does not have to be completed for emergency or temporary placements.
- At any time the placement of a child in out-of-home care is changed.
In completing the Placement Assessment tool, the social worker:
- Opens the case in CWS/CMS.
- Opens a new Placement Assessment Tool.
- Fills in demographic information not auto-populated from CWS/CMS.
- The Date of Initial Placement refers to the date of the child's first placement in out-of-home care, when initially removed from the parents or guardians.
- Line D is reserved for information regarding substitute care providers.
- To protect confidentiality, write in the foster parent's name using first name and initial of last name and an abbreviation for the facility type.
- Clicks Start/Resume WIZARD to complete the tool.
- Completes fields under:
- Participants Not Assessed, if applicable
- Substantiated Allegations
- Question 1 through 17
- TDM
- Document Review
- Types in his/her signature and the date on the signature and date lines.
- Saves the tool online by clicking the Exit and Save Document button at the lower right of the WIZARD Main Menu.
Supervisor:
- Opens and reviews the Tool in CWS/CMS.
- Types in his/her signature and date on the signature and date lines at the end of the Tool to indicate approval.
- Saves the Tool by clicking on the Form Completed, Save for Analysis button at the bottom of the Wizard Main Menu, after approving the Tool.
