| Purpose of the Case Closure Tool |
|
The Tool is to be used to determine whether:
- The child is in a safe and permanent home.
- The social worker can safely terminate child welfare involvement.

| Who Completes the Case Closure Tool |
|
The social worker assigned responsibility for the ongoing case management of a case completes the Case Closure (CC) Tool. This includes:
- DI social workers
- VFM social workers
- IS social workers
- Continuing social workers

|
Refer to page 95 of the CAT User Guide.
- One form is completed for each child in the case.
- The Case Closure (CC) Assessment Tool is to be completed when the case plan goal for the child has been achieved.
- Completion of the CAT - CC Tool is not necessary when closing a referral.
- The CAT - ER Tool is used when closing referrals.
In completing the Case Closure Assessment Tool, the social worker:
- Opens the case in CWS/CMS.
- Opens a new Case Closing Tool.
- Clicks Start/Resume WIZARD to complete the Tool.
- Fills in the yellow field of the Assessment Date.
- Completes questions under each of the categories numbered 2 through 12.
- Include a narrative statement under category 12, Comments/Closing Summary, that indicates the steps taken to link the family to services and specifies the aftercare or follow-up plan, if one is developed.
- If copying a closing summary from another document, the Control V function must be used to paste the text into the narrative box
- Types his/her name and the date on the signature and date lines at the end of the Tool.
- Saves the Tool online by clicking the Exit and Save Document button at the lower right of the WIZARD Main Menu.
- The Tool is saved in CWS/CMS for future reference.
Supervisor:
- Opens and reviews the Tool in CWS/CMS.
- Saves the Tool by clicking on the Form Completed, Save for Analysis button at the bottom of the Wizard Main Menu, if approving the Tool.
- Types his/her name and date on the signature and date lines at the end of the Tool to indicate approval.
