Ambulatory Care

Problem Statement

Community Health Centers (CHCs) are well known and trusted fixtures within communities where the hard to count populations reside.  Therefore, CHCs are an ideal entity/location to support Census outreach and education and to serve as a safe location for members of the community to complete their Census questionnaires.  While Clinics may be willing to serve as a Census center, where the community frequently visits, CHCs do not have the personnel or resources to carry out Census completion related activities due to the need to focus on clinical and other services for their patients.  That being said, participating CHCs could disseminate Census outreach and education material if provided with in-language outreach material, designated Census completion staff, and equipment (kiosks and appropriate devices) to undertake these important and specific functions within the health center environment.
