Data may be incomplete: In early August, it was discovered that there was a reporting issue with the State’s system that affected all counties. While the State believes this issue was resolved, we continue to validate our data.
This dashboard provides information on cases among residents and staff at LTCFs, which are a critical part of the
continuum of health care. LTCFs include skilled nursing, independent living, assisted living and board and care
facilities. Because LTCFs typically serve older individuals with chronic health conditions, residents of LTCFs are
at greater risk of severe disease and death from COVID-19. Given this, the Public Health Department pays very close
attention to these facilities with a specialized team focused on responding to LTCF cases and potential outbreaks.
Facilities may access testing through commercial labs or academic labs, as well as the Public Health lab. The Public
Health Department facilitates testing of all symptomatic residents and staff of skilled nursing facilities and other
licensed facilities. When there are concerns about a possible outbreak, transmission in a facility, or risk to
adequate staffing, the Public Health Department initiates an investigation into the facility, which may include
additional symptom screening and more extensive testing. When there is a positive case identified in a skilled
nursing facility in Santa Clara County, the Public Health Department identifies all individuals who may have come in
contact with the case and the duration, frequency, and proximity of that contact to identify possible exposure to
staff and residents. Guidance has been issued to all LTCFs facilities on COVID-19 control and prevention.

is collected. Only LTCFs within Santa Clara County are listed. Residents and staff working at these facilities
and who are residents of Santa Clara County are included in these counts. Hospitalizations are only determined at
the time a case was reported to the Public Health Department and do not represent patients that are currently or
have ever been hospitalized.
