5150 Information

5150 refers to an involuntary evaluation. Section 5150 is a section of the California Welfare and Institutions Code (specifically, the Lanterman–Petris–Short Act or "LPS") which allows a qualified officer or clinician to involuntarily confine a person deemed to have a mental disorder that makes them a danger to him- or herself, and/or others and/or gravely disabled. Attached are current lists of Behavioral Health Staff that are authorized to issue a 5150 hold.

Learning Partnership is implementing a new registration process for those wanting to attend the Clinical Aspect of 5150 training. This new sign-up process will help expedite approving your application. Effective July 18, 2016,  fax applications will no longer be accepted, applications need to be emailed to:  LP5150@hhs.sccgov.org

The following is required to be included with the application:
 
  1. A copy of your current license; 
  2. The attestation form;
  3. The online breEZe license verification from: https://www.breeze.ca.gov/datamart/loginCADCA.do (click on Verify a License).
  4. Applications need to be submitted ten business days before the desired training date and must be sent to LP5150@hhs.sccgov.org.
