California Children's Services, or CCS, provides case management and funding for the diagnosis and treatment of qualifying medical conditions. Additionally, it provides physical and occupational therapy at Medical Therapy Units. The Public Health Department operates three Medical Therapy Units in Santa Clara County. Nurse case managers assist families in navigating the health insurance system and coordinating care with specialists and children’s hospitals, as well as transitioning to adult care.
