Child Health and Disability Prevention (CHDP)

The County of Santa Clara Public Health Department operates our county's Child Health and Disability Prevention program. The Child Health and Disability Prevention program, or CHDP, is funded by the state of California and provides for the early detection and prevention of health problems among children and youth from low-to-moderate-income families. The program provides reimbursement and training to medical providers. Participating providers receive monitoring and support. Our CHDP program staff work collaboratively with health care providers and child advocate agencies to ensure that eligible children receive quality health care and to empower families to be knowledgeable, responsible health care consumers.

Children and youth are eligible if they are:

  • On Medi-Cal and 0-21 years old
  • Low to moderate income and 0-19 years old

Children and youth may be able to receive temporary Medi-Cal for up to 60 days through the CHDP gateway.
