Tobacco Prevention Resources for Healthcare Providers
The Santa Clara Public Health Department's Center for Chronic Disease and Injury Prevention (CDIP) is dedicated to promoting strong partnerships with local healthcare providers to reduce chronic diseases within our local community. Through recent federal grants from the Centers for Disease Control, CDIP has been able to access national experts to increase resources and tools to help reduce tobacco use and exposure.
Funding also promoted unique partnerships and opportunities to reach vulnerable populations who have higher rates of smoking, such as mental health patients and Vietnamese community members. Together we can help to reach more of our community members who are addicted to nicotine. Please utilize the resources below to help reach your patients and help them to quit smoking. INSPIRE Tobacco Cessation Network
The INSPIRE Tobacco Cessation Network is a unique collaborative in Santa Clara County which provides cessation services in multiple languages including outreach, referral, one-on-one counseling, over the phone counseling, group counseling, nicotine replacement therapy, educational materials, and follow-up services.
