Child Health and Disability Prevention (CHDP)

Our mission: The Santa Clara County Child Health and Disability Prevention (CHDP) program is dedicated to the health and well being of children and youth. CHDP provides for the early detection and prevention of health problems among children and youth from low-to-moderate income families. Program staff works collaboratively with health care providers and child advocate agencies to ensure that eligible children receive quality health care and to empower families to be knowledgeable, responsible health care consumers.
Children and youth are eligible if they are:
  • On Medi-Cal and 0-21 years old
  • Low to moderate income and 0-19 years old (Children and youth may be able to receive temporary Medi-Cal for up to 60 days through CHDP gateway

The “Access to Oral Health Care” survey, created by the Santa Clara County Child Health and Disability Prevention (CHDP) program, was administered to local Medi-Cal accepting dentists, as well as CHDP and Comprehensive Perinatal Services Program (CPSP) participating providers in Santa Clara County.
The surveys, which were tailored according to each of the three participating groups, were sent out in May 2014, to a total of 107 CHDP providers, 148 dentists and 23 CPSP providers. Download the report >
