- Cab company or owner must confirm appointment through email within 24 hours. Failure to confirm within 24 hours will cancel the appointment.
- Any changes must be emailed using this form and approved by W/M before it becomes effective.
- Please check-in with reception to retrieve your badge, prior to your inspection. We do not accept late check-ins.
- Cab company or owner will be charged a $50.00 missed appointment fee if cancelation is less than 24 hours prior to the appointment or driver is later than 5 minutes to check–in and/or inspection. Companies scheduling owner/operator appointments are responsible for those appointments.
- Cab owners will be charged a $70.00 re-inspection fee after a failed inspection.
- Cab owners will be charged a $70.00 re-inspection fee if requesting an appointment within 10 months of their last inspection.
